Empowering people with disabilities to live independently in the community

Our staff specializes in identifying needs of people with disabilities to make it possible to live in a home of their choosing. We continually develop community partnerships that work towards creating opportunities for people to thrive in an inclusive community.

HOME Choice Fact Sheet

Established in 2008 with a goal of transitioning 2,000 Ohioans, the program has become a national leader in moving people back home. As of July 1, 2019, more than 13,000 people have enjoyed a new-found independence though HOME Choice.
