Empowering people with disabilities to live independently in the community

Our sta­ff specializes in identifying needs of people with disabilities to make it possible to live in a home of their choosing. We continually develop community partnerships that work towards creating opportunities for people to thrive in an inclusive community.

As a Center for Independent Living, one of our core services in promoting and advocating for community-based living options. Ohio’s HOME Choice program transitions eligible Ohioans from institutional settings to home and community-based settings, where they receive services and supports at home and in their communities.

If appropriate for the HOME Choice Program, Ability Center staff will connect an individual to the HOME Choice Program and assist them throughout the application process.

Who is eligible to participate?

To be eligible for HOME Choice an individual must:

  • Be enrolled in Medicaid
  • Have resided in a long-term care facility for at least 90 consecutive days
  • Be 18 years of age or older
  • Have income to sustain community living
  • Participate in a needs assessment and have a need for the program
  • Have care needs that can be adequately met in a community setting
