Nursing Home Transition Program

Are you ready to leave a long-term care facility and move back into community living? We can help with this transition.

The Nursing Home Transition Program connects people with disabilities to programs and services necessary to maintain independence in the community. Our staff act as Transition Coordinators for the Ohio Department of Medicaid’s Home Choice Program. If you are ready and able to leave a long-term care facility, our staff will assist you in coordinating your transition.

Individuals must reside in a nursing facility for a minimum of three months before they can qualify for the Ohio Home Choice Program. They must be on Medicaid, show proof of income, and should have a support system in the community. An individual or nursing home staff person can call and speak with someone on the Nursing Home Transition team for more information concerning the transition process.

Besides having the goal of building an inclusive team, what I hope to leave behind during my time with the Toledo Crash is to change the societal perspective of what it means to be a disabled athlete.
