Agency Applications

Ability Center College Scholarship – application form for the center’s scholarship program.
Board Nomination – form used to nominate individuals for service on The Ability Center’s Board of Trustees.

Housing Accessibility Program Application – application used to obtain a ramp or other home accessibility modification from the center.
TARPS-TARTA Complaint Form – this form is available to assist you with filing a complaint with TARPS or TARTA.
