Summary of Program
The Nursing Home Transition Program assists people by connecting them with programs and services necessary to maintain independent living within a chosen community.
We are Transition Coordinators for The Ohio Department of Job and Family Services’ “Access Success” and “Home Choice” programs.
If you are ready and able to leave a long-term care facility setting, the Nursing Home Transition staff is ready and able to assist you in coordinating your transition.

Home Choice Newsletter Spring 09 Cover
The Ability Center has been recognized twice in the Spring 2009 “Home Choice News Bulletin”.
First, for its effort assisting consumer to achieve her dream of having her own home.
Second, ACT is thanked and congratulated by ODJFS for creating the Home Choice Database. They further encourage and offer the database to other agencies as a management tool.
