ACT Reasonable Accommodation Request Letter-01-09 – This form begins the process of seeking advocacy support when faced with discrimination on the basis of access.

TARPS Application-05-09 – This is the Official TARPS Application to receive service.

Senior Emergency Cell Phones-03-09 – Use this application to apply for a cell phone through the Emergency Phones for Seniors Program from the Lucas County Commissioners and the Area Office on Aging.
