The Ability Center provides the forms listed on this page for your convenience. The forms are in  an accessible PDF format. Agency Applications

Ability Center College Scholarship – application form for the center’s scholarship program.

Board Nomination – form used to nominate individuals for service on The Ability Center’s Board of Trustees.

Camp Cricket Application – form to participate in annual summer day-camp.

Employment Application – application for those seeking employment at the center.

Housing Modification Program – application used to obtain a ramp or other home accessibility modification from the center.

TARPS-TARTA Complaint Form – this form is available to assist you with filing a complaint with TARPS or TARTA.

Volunteer Application – form used to apply to participate in volunteer activities at the center.

Community Resource Applications

Senior Emergency Cell Phone Services – form used to apply for cell phone services through the Lucas County Commissioners and the Area Office on Aging of Northwest Ohio.

TARPS Transportation – for used to apply for services from the Toledo Area Paratransit Services (TARPS).
